Welcome to the Nameplate Badge Order Form! To get started, please fill out the following fields, and please address each field in full detail to avoid any delays with processing your order!

Nameplate Badges start at $65. Price includes shipping costs* and character with or without simple clothing. Price increases for requests for any detailed clothing/accessories, intricate tattoo/marking designs, and/or the inclusion of props/pets. Please include requests for any of these items in the last field in this form, and you will be contacted with different price options before receiving your invoice.
